Through close communication with our residents and their loved ones, we develop and provide a person-centred care and support plan tailored to suit everyone's unique, specific needs. We make sure this care plan is regularly reviewed and adapted when necessary.
When it comes to providing the best care possible for your loved ones, attention to detail is so important. Nourish is a digital care planning system that allows our staff to carefully manage and monitor every aspect of a resident’s care plan, ensuring that nothing is missed, and that they receive the support that they deserve.
That's why we can proudly say that all our homes are rated as Good or Outstanding with the Care Quality Commission.
Making the Transition Easy for You and Your Loved One
Moving into a care home can be challenging, and this is why each of our staff members focus on recreating the comfort of a new environment as best as we can. It all starts with encouraging residents to bring their own furnishings and decorations for their living spaces to help them feel comfortable in their new environment. We’ll also make sure we provide a great range of freshly cooked, healthy meals for each of our residents, catering for all dietary preferences.
Our activity co-ordinators also take the time to understand every resident’s interests and hobbies, allowing us to provide a bespoke range of activities and events to ensure that your loved ones can continue doing what they love with us.
Part of the Community at Black Swan and Beyond
It is key for us that our residents can continue living their lives as part of the community. By ensuring our residents have plenty of opportunities to visit local attractions, cafes, social clubs and pubs, we allow them to maintain close friendships with the local community.
We do this by nurturing relationships with local schools, churches and children’s groups, as well as holding regular open days, summer fetes, coffee mornings and afternoon teas to promote further engagement with the community.
